Stopping Weight-Loss Medications: How It Compares to Quitting Your Diet

For those who use weight-loss medications like semaglutide (Wegovy) and tirzepatide (Zepbound) see significant results- patients can lose up to 15-20% of their body weight. Not only does the number on the scale decrease, patients often see improvements in metabolic markers such as decreased blood sugar, blood pressure, and cholesterol. But a common and important question remains: What happens after you stop taking the weight loss medications?
The simple answer: most patients regain the weight they lost.
But how does this compare to those who only made lifestyle modifications through diet and exercise and didn’t use weight loss medications?
A recent study of over 9,000 participants published in the British Medical Journal examined what happens stopping a weight loss regimen. First, the study compared whether there was a difference between stopping a semaglutide versus a tirzepatide. There was no meaningful difference between the two groups of medications. On average, they regained about one pound per month whether they had discontinued semaglutide (Wegovy) or tirzepatide (Zepbound). Within 1.5 to 2 years, most people regained nearly all the weight they originally lost. Along with this weight re-gain, the initial metabolic improvements decreased.
However, people regain weight much more slowly after lifestyle programs compared to those using weight loss medications. After stopping lifestyle programs, participants only regained an average of 0.25 pounds per month. This shows that while weight loss medications lead to greater weight loss upfront, that weight is regained more quickly once the medication is stopped. So while medications lead to greater weight loss upfront, the weight often comes back faster once the medication is stopped.
Weight-loss medications can lead to meaningful weight loss and improved health, but their rate of weight regain is higher than for those who are not using them. This highlights the importance of long-term care, realistic expectations, and ongoing support when treating obesity.
